Our clinic is paperless and does not have a receptionist so all of our forms are digital and will be sent to you via email. We are not permitted to have patients fill in forms at the clinic at this time due to the fact that we have been mandated to limit time patients spent in the clinic outside of the actual treatment. We are requiring that all patients fill in all forms prior to their appointment so we can be compliant. By filling in your forms ahead of time you are not only ensuring you that you get more time in treatment, better quality care, you are also preventing unnecessary work on the part of the therapist. Your appointment will be cancelled if you do not fill in your forms.
If you are sent an email that says that you have a form to fill in trust us you do. Please just click the fill out intake form and fill it in. We have various forms that are sent out the same way. So even if you have filled in a form previously and the forms looks the same trust us its a different form and you need to fill it in. The link for all forms expire 24 hours from the time the email is sent. This is to protect your privacy. In the event that you do not fill in the form before the link expires you can log into your account to fill it in ore mail the office at email@example.com and we will resend it.
We also have a few required forms so if its your first appointment ( or your first appointment in a while) you will have multiple forms to fill and by filling them in when you receive them it prevents the therapists from having to go into your profile and manually send out forms that are required.We try to make things as seamless as possible for you so that any redundant information is automatically filled in for you however we will still need you to fill in the forms.
Here are all the forms that apply to you:
Health History Form: this form is required for all patients and you are sent this automatically on your initial appointment for each modality. S0 even if you filled one in for massage therapy and now you are booking an acupuncture appointment you will need to fill in another form. For massage therapy we are required to have updated health history forms on file for each patient. This means that every two years you will be required to update your health history form. Most of the fields will already be filled in you will just have to update information.
COVID Liability Form: due to the COVID pandemic we require all patients to sign a COVID liability waiver prior to their first appointment. We are not willing to treat patients that do not fill in this form.
Mandatory COVID screening: no longer required. Please still assess for signs and symptoms of communicable disease and cancel appointments if you are sick.
Motor Vehicle Accident form: If you have been in a motor vehicle accident (even if you are an existing patient) you will be required to fill in a motor vehicle accident form. If you have been in a motor vehicle accident you will need to book the initial MVA appointment for this form to automatically be sent. If you have been in a motor vehicle accident and you did not receive the form please email us at firstname.lastname@example.org and we will re-send it to you.
